The applicant of apply for an ID card- The applicant shall file the application with the registry office where his or her permanent residence is located.
- For applicants below fourteen years old, this shall be done by their legal representatives.
Documentary proof that applicant should prepare - Initial receipt: Seal (or signature) of party concerned or legal agent , identity card of the legal agent.
- Replacement: personal old national identity card, seal (or signature). If old card is seriously damaged, photo cannot be identified or photo is blurred, process compared to supplementary receipt method.
- Supplementary receipt: personal seal (or signature), approved and effective certifying document with photo issued by government institution (can shorten processing time)
- 3 x similar half body, front, hatless and glossy color photos taken by the party concerned within the latest 2 years (vertical 4 cm , horizontal 2.8 cm or 2 inches).
- Applicant's seal, national identity card.
- Government fee: Initial card replacement $50, supplementary card $200.
