The applicant of death/death announcement registration - The spouse, relative, head of family, cohabitant, the person handling burying, or the managing person of house or land of the dead when he/she died.
- The person applying for the announcement or the interested person.
- In the case of execution or death in jail, detention house or other shelters, and no one claims the dead, the jail, detention house or shelter shall notify the census office in the place where the domicile of the dead was to make the Death Registration.
- In case of death due to disasters or the dead being unidentifiable, or investigated and verified by police department but no one claims the dead, the police department shall notify the census office in the place where the domicile of the dead was to make the Death Registration.
- The depository.
Documentary proof that applicant should prepare- Household registry.
- National identity card of the deceased and its spouse.
- Applicant's seal, national identity card.
- Death certificate, examination report or corpse examination certificate, for death declaration attach with letter of -judgment from court.
- Letter of consignment.
